Please note that CareStar Transportation has contracted with Medstar Transportation in Washington State to facilitate
administrative services for the Company. Despite this arrangement, your direct affiliation will be with CareStar
Transportation in Arizona. Our Company is NOT affiliated with Medstar Medical Transport in Arizona.
